It is with great pride that I am here to report that, due to recent changes in the structal fabric of the multi-versal budget, a Class 5 prior-authorization form will no longer be required to be submitted with non-emergent applications for obtaining authorization request status disclosure. A Class 4, Class 4B, or Waiver CZ-409 will be recognized as acceptable verification for such requests.
Emergent applications must still obtain Class 5 or higher authroiuzation.
